The Immunization of School Pupils Act (ISPA) requires families and caregivers to ensure their children receive specific vaccinations. The ISPA also provides authority for Public Health Units to issue school suspension orders, which schools are required by law to enforce.

Region of Waterloo Public Health has mailed suspension notices to families of secondary school students who are not up to date on their immunizations. Families and caregivers need to ensure their child’s immunizations are up to date and reported to Public Health to avoid a suspension under the ISPA. ISPA-related suspensions will begin for secondary students on May 1, 2024.

Families and caregivers of students who have received suspension notices are encouraged to visit the Public Health website to update immunization records and/or schedule appointments for necessary vaccinations for secondary students.

Please note that Public Health is unable to release health information to families or caregivers for students over the age of 16 years without their consent.

Before you make your appointment

Before booking an appointment, families, caregivers or students should update students’ immunization records. It is important to ensure that all vaccinations your child received from your healthcare provider or out of province have been reported to Public Health.

Report immunization records

A student’s Health Card number is needed to view their immunization record. If you do not have a health card, you can use the Ontario Immunization Identification (OIID) usually found on a child’s report card.
