Renewal Documents

The following documents are required, along with the Student’s current Study Permit, as part of the renewal process for International Students:

  • Parent Letter of Permission  
    • Must be signed by the parent(s). The form indicates that the student wishes to continue with the Waterloo Region District School Board and confirms custodian information.  We cannot accept a digital signature on this form.
  • New Student Agreement
    • Student and custodian read and then sign the document. We cannot accept a digital signature on this form. Please sign with a pen.
  • Consent for Information – Age of Majority Form
    • If the student turns 18 during the school year, the student must complete and sign this form. We cannot accept a digital signature on this form.
  • Custodian Appointment Letter and Custodian Agreement Letter
    • If custodian information changes, we require a new notarized Custodian Appointment Letter and a notarized Custodian Agreement Letter
