The message below is from the Region of Waterloo Public Health, regarding immunization notices.

On February 6, 2013 approximately 4100 notices (questionnaires) were sent out to homes of elementary students with incomplete records.  This notice will request that complete immunization records be provided to Public Health.

We ask that if students or parents have any questions regarding these notices that they be redirected to Public Health at 519-883-2007 and press option 6.  Alternatively, they can drop in to our public health office at 99 Regina  Street South in Waterloo, or 150 Main Street in Cambridge, during business hours.  Information about immunization and submitting records can be found at our website (www.regionofwaterloo.ca/immunization)

With appropriate legal documentation, exemptions based on medical, religious or conscience reasons are allowed.  If students fail to provide the immunization record or legal exemption to public health, they may face suspension from school for up to 20 days or until the  documentation is completed and received.  Suspension day this year is March 26th for elementary students.
